Frequently Asked Questions

  • At 412 Dance Medicine we are not limited to what insurance companies authorize. You and your physical therapist make the decisions on what is best for you.

    There are no visit limits and you will get 1:1 attention you deserve from your PT. Our PTs are not rushed and can spend the time to get to the point of your pain.

  • 412 Dance Medicine will provide you a "superbill" upon request that you can submit to your insurance.

    Most insurance carriers have "out of network" benefits. Please check with your insurance as each plan is different.

  • In the state of Pennsylvania, PTs who have a direct access license can treat patients without a physicians referral or 30 days. If you would like to continue after the initial 30 days, we will need a prescription to continue with physical therapy.

  • These dance specific sessions are aimed at healthy dancers who want to improve their performance. They are not medical sessions and therefore not eligible for reimbursement by insurance.

  • At the initial session you and your PT will discuss your medical history, relevant previous injuries, and your current reasons for seeking physical therapy.
